Forms and Fees Days

Below is basic information regarding Forms and Fees Registration Days for the 2017-18 school year.

Dates/Times/Location:

August 10: 12-7pm at Shorewood High School Arena
August 11: 7am- 2pm at Shorewood High School Arena

ALL Lake Bluff, Atwater, Shorewood Intermediate School, Shorewood High School and New Horizons for Learning Charter School (Charter school at SHS) families will come to this location. Registration does not occur at individual schools.

STEP 1: ONLINE


Please note: The online portion will need to be completed before going through the in-person portion.


Part 1- Online Registration through Infinite Campus:

All families must first complete Online Registration, which you can access via the Parent Portal within Infinite Campus. The website address for the Parent Portal is https://wicloud3.infinitecampus.org/campus/portal/shorewood.jsp There is also a link on the district’s homepage. Look for the green box located on the left side of the page labeled “Infinite Campus Parent Portal.” Click on the box and it will take you to the login page. Once you’ve logged in, you will see a list located on the bottom-left-side of the page with a link that says “Online Registration.” Follow that link. If you have misplaced your login credentials for Infinite Campus, please contact Meghan Anker in the Pupil Services office at 414-963-6903.

Online Registration in the Parent Portal goes live for families on July 7, 2017. Please ensure all of your demographic information is accurate and up-to-date, as your child’s teacher and building administrator will utilize this information frequently over the course of the school year. You will also need to complete annual release agreements and permissions.

Please note: The online portion should be completed before coming to Forms and Fees Days. If you do not have access to a computer or internet, computers will be available at Shorewood High School to complete the Online Registration portion on-site. Employees will also be available if assistance is needed.

STEP 2: IN-PERSON

Part 2- In-Person Residency Verification:

All families must attend one of the Forms & Fees days to verify residency. In accordance with Wisconsin State Statute, all school districts are required to verify residency for ALL students attending their schools on an annual basis. This applies to both resident and non-resident students (Chapter 220, Open Enrollment). Per Shorewood School District Policy #420, Shorewood School District families need to provide one piece of documentation from EACH of the following lists annually:

List 1:
a) Current property tax statement (readily available at Village of Shorewood website)
b) A closing statement if you newly purchased a home (within the previous year)
c) A signed current resident lease (including the landlord’s name, address, and phone number)

List 2:
a) Current month’s utility bill
b) Health insurance statement
c) Food Share/Quest benefit statement
d) Medicaid/Badgercare benefit statement
e) W-2, SSI, or other country, state, or federal benefit statement

These documents should be brought to Forms & Fees Day on either August 10th or 11th. The documents will be scanned and immediately returned to you. Please note: You will not be able to complete the registration process until Residency Verification has been completed. If the required documentation is not submitted, you will be asked to come back once it has been obtained.

After you’ve completed Online Registration and Residency Verification, you will be able to do the following at the Forms & Fees Event:

  • pick-up student class schedules/teacher assignments
  • SIS & SHS students will get their pictures taken for their school ID and yearbook
  • speak to the District Nurse and turn-in any applicable medical forms
  • receive pertinent information from your school’s PTO
  • apply for Free or Reduced Lunch through the online FRAM application
  • pay applicable school fees

FORMS AND FEES EVENT INFORMATION

Location: Shorewood High School Arena

Dates/Times: August 10th: 12:00pm – 7:00pm, August 11th: 7:00am – 2:00pm

***All Lake Bluff, Atwater, SIS, SHS, and New Horizons Charter School families will come to this location.***

School Fees will be available to pay online as of August 1st. Credit cards and eChecks are accepted through Infinite Campus with no convenience fee. You will have the option to be able to pay school fees at the Forms & Fees day as well. Additional information will be sent via an email communication on August 1st.

If you have any questions or concerns related to Online Registration or the Forms & Fees Event, please contact the Pupil Services Office at 414-963-6903. If you know that you will NOT be able to attend either of the two dates for Forms & Fees Day, please reach out to the Pupil Services Office to set-up and appointment for residency verification (appointments can be scheduled now but will take place on or after August 14th). Online registration and residency verification must be completed prior to the start of school to ensure there are no delays with your child starting classes on September 5th.

FREQUENTLY ASKED QUESTIONS

I am not available to attend either Forms and Fees Day to complete the in-person portion of registration, what are my alternative options?
We know that a number of families will not be available to complete the in-person portion of enrollment on those days due to previously scheduled events. I have no idea where my tax bill went; I paid it back in January.

It's easy. You can download and print your most recent property tax bill from the Village website:http://gcs.villageofshorewood.org/search.aspx You do not need to know any information other than your street address in order to locate your bill.


I will be living at a Shorewood residence beginning September 1. What should I do?

Bring whatever documents you have to show your new residence (lease or closing statement). You will be asked to sign an affidavit indicating the date you will move in, and we will follow up at that time to receive the other required documentation.


Help! I don't have a formal contract with my landlord.

Please contact your landlord to create an agreement. It protects your rights as a tenant, and the District as proof of residency requires it. You can work with your landlord to legitimize the status of your arrangement. A standard rental agreement form is available for about $15 from Wisconsin Legal Blank.


What if the home or lease is not in my name?

You will be required to provide us with signed and notarized "Verification of Residence" forms if your home or lease is not in your name or if your child is living with a relative. Be advised: per the Thayer Rule (http://oe.dpi.wi.gov/residency), your child will not be enrolled in a Shorewood school if the sole reason for residing in Shorewood away from a parent/guardian is to attend school.


I live in Shorewood, but my child's other parent does not. My child only stays with me on certain days of the week. Can I enroll?

Yes, as long as the student under a joint custody order does live part of the time with both parents and either parent resides in Shorewood. The Wisconsin Department of Instruction does NOT require that the student reside a certain percentage with the resident parent. Both of your child's addresses must be in school records.



Does my cell phone bill count?

We will not accept cell phone carrier statements, because cell phones are not necessarily tied to a particular address. A bill from a telecom company is acceptable only if it includes residential service (aka land line) or other site-based services such as cable TV and Internet.


I pay my utility and insurance bills online. Can I give you a printout from a pdf or website?

Yes, but the statement must include the required pieces of information (name, address, and date as specified above).


I already enrolled my child to start kindergarten in the fall. Do I have to bring my documents again to be scanned?

Yes.


I do not have a printer.

If you do not have access to a printer, you can use a printer at school when you come in on Forms & Fees Days. On those days, the Shorewood High School computer lab will be open and staff will be on hand to assist you. Please note that the Shorewood Public Library also has computers and printers available for public use. Please do not come to the scanning station without papers ready to scan since you will not be able to continue the enrollment process until this portion is completed.
