To protect students from diseases that are preventable by immunization, Wisconsin law requires all students to show that they have received the required immunizations or have a signed waiver. Below you will find links to information about the law and materials that schools, day cares, and public health agencies use to implement the law ( dhs.wisconsin.gov).

Wisconsin Immunization Registry
You may access your student's vaccination records here using their name, date of birth and SSN or Medicare number.

Waivers
For those that do not vaccinate due to religious, medical or personal reasons, please complete sections 1, 4 and 5 of the student vaccination record sheet. You must check all vaccinations you are waiving .

Waiver - English
Waiver - Spanish
Waiver - Hmong

Completing a waiver does not guarantee your child will not be excluded from school should an outbreak of the illness occur in the school. This exempts you from legal action by the State of WI only .

Student Immunization Law Age/Grade Requirements
Although some vaccinations are not required by the State for entrance to public schools, the Center for Disease Control (CDC), Wisconsin Department of Health Services and American Academy of Pediatrics strongly recommend students receive the following vaccinations (*required) :

Childhood

  • Hepatitis B*
  • Rotavirus
  • Diphtheria, tetanus and pertussis (DTaP)*
  • Haemophilus influenzae type b (Hib)
  • Pneumococcal
  • Inactive poliovirus*
  • Measles, mumps and rubella (MMR)*
  • Varicella (Chicken Pox)*
  • Hepatitis A

Adolescent ( Ages 11+ Fact sheet)

  • Meningococcal/Meningococcal B
  • Tetanus, diphtheria and pertussis (Tdap)*
  • Human papillomavirus (HPV)

Annual (All ages)

  • Influenza (Flu Shot)

ALLERGY SEASON IS HERE
We have seen a huge increase in health office visits from students with complaints of red-itchy eyes, stuffy-runny noses and sneezing. Being outdoors more often for lunch, recess and classroom activities means an increase in students coming to the health office.

We are unable to provide allergy medications or eye drops to the students.

If you think your student has seasonal allergies or has been complaining of these symptoms, consider an over the counter allergy medication or eye drop. Please avoid using Benadryl as this medication can make students very sleepy. If you must use Benadryl, please administer it after school or before bedtime instead of in the morning before school.

If you are unsure of which medication to use, please talk to your child's provider to see what is the safest option for your child. If you have any further questions, please contact your school's health team.

EPINEPHRINE AND INHALERS
If your child has a life-threatening allergy and has been prescribed an epinephrine auto-injector (EpiPen, Auvi-Q), please ensure this medication is with your child at school. The school supplied epinephrine auto-injectors are limited to 2 auto-injectors for each school. This means that school supplied epinephrine auto-injectors cannot be sent out on all field trips, especially towards the end of the school year when many classes are going on trips each day.

Inhalers are not supplied by the school district. If your child has asthma, please be sure to send their inhalers to school, especially if your child will be going on a field trip (this includes walking field trips). If a child is experiencing asthma-like symptoms and the child's inhaler is not with them, emergency services will be contacted immediately.

MEDICATION - END OF THE YEAR
Parents/guardians - please plan to pick up your child's medication on June 9, 2022. Controlled substances will NOT be sent home with your child at any age. These must be picked up by an adult. Students in 7th-12th grade with over-the-counter and prescription medication (not classified as a controlled substance) may pick up their medication at the end of the school day on June 9, 2022. Students in Elementary grades cannot collect medication to bring home.

Sincerely,
