Sports physicals are good for two years. Please have your doctor fill out the following document when you get a physical:

• Physical Waiver

Alternate year cards are needed on non physical years. If you had a physical last school year, parents/guardians should fill out and return the following document.

• Athletic Association Alternate Year Athletic Permit Card
