  • 2025-2026 District Medication Form
    • For all prescribed medications that your child may need to be given at school, this form must be filled out completely with both a guardian and prescribing provider signature and turned into the school's health office.
    • Students may not self carry any medications, over the counter or prescribed, at school. 
      • The only exception to this is if a prescribing medical provider has filled out the medication form stating a student may self carry an emergency/rescue medication. 

 

 

  • Allergy Action Plan Form
    • Please do not use this form for food/medication sensitivities/intolerance. This form is only used for allergies that cause anaphylaxis and require epinephrine (Epi-pen, Auvi-Q, Neffy).

 

  • 2025-2026 HIPAA Release
    • This document is for the release of medical information from your child's medical provider to the school district. This may include, immunization records, or records related to illness, injury or diagnosis. If you have records you would like the district to receive directly from your child's medical provider please contact the district nurse at 414-961-2888.
